Cranial Osteopathy or Shiatsu?

Understanding the Difference — and Choosing What Your Body Really Needs

One of the questions I’m asked quite often in my clinic here in Cheltenham is:
“What’s the difference between cranial osteopathy and shiatsu — and how do I know which one to book?”
It’s an important question. And it’s one I can answer from personal experience as well as from over 20 years as a shiatsu practitioner.
I received cranial osteopathy for more than ten years. It helped me enormously and I still believe it is an exceptional therapy. But eventually I discovered that one particular issue - my persistent knee pain was resolved with a different therapy.
It turned out to be shiatsu.
This article isn’t about choosing one over the other. It’s about understanding what each therapy does well — and recognising when your body may need a different approach.
My Experience with Cranial Osteopathy
Cranial osteopathy, is a more subtle version of traditional osteopathy and deeply skilled branch. It works with gentle touch to support the natural rhythms of the body; particularly the cranial bones, sacrum and cerebrospinal fluid system.
From my own experience receiving treatment, cranial osteopathy can:
Calm and regulate the nervous system
Ease headaches and jaw tension
Support recovery from stress or trauma
Improve overall structural balance
Leave you feeling deeply settled and restored
It works profoundly with the body’s inherent intelligence. For many people particularly those who are sensitive, anxious or overwhelmed; it can be life-changing.
It certainly was for me as over the years I realised it did so much for my emotional balance.

When the Missing Piece Is Muscular Tension
Despite years of cranial treatment, I continued to experience knee pain. I was told at the age of 5 it was 'growing pains' and it still impacted my life into my early 30s making long distance driving or going to the theatre/cinema unbearable.
I happened on shiatsu when I was looking for a therapeutic massage; one that would help me emotionally. I was 100's of miles away from my usual cranial practitioner and the one's I'd gone to in London just didn't match up. Whilst receiving this incredibly gentle therapy a positive side effect was the elimination of life-long knee pain - it was incredible!
Eventually I realised the issue wasn’t primarily structural or cranial in origin : which had all be resolved prior to this. It was muscular. My pelvis/hips/knees were being pulled by chronic tension patterns and the tissues simply weren’t releasing.
When I began receiving shiatsu, that changed.
The sustained pressure and stretches worked directly into the muscle and fascia. Circulation improved and long-held tension softened. Lifelong cold hands and feet naturally started to be warmer and I could actually kneel without pain!
That experience shaped how I now explain shiatsu to my clients who visit me for treatments from across Cheltenham, Gloucester and the wider Cotswolds area.

What Shiatsu Actually Does (In Clear, Practical Terms)
Shiatsu has its roots in East Asian medicine and you’ll often hear the word “Qi” used. Rather than framing this in abstract terms, I prefer to explain it in ways that make physiological sense.
When we say shiatsu “moves Qi”, what is observed in the body is:
Muscles releasing
Fascial layers gliding more freely
Improved blood and lymph flow
Reduced joint compression
The nervous system shifting into repair mode
Shiatsu uses steady, sustained pressure along anatomical and energetic pathways that closely mirror major muscle chains and connective tissue lines.
In very grounded terms, it helps:
Release chronic muscular holding
Improve fluid movement
Restore elasticity
Reduce pain caused by tension
Rebalance stress patterns stored in the body
For many people I see their pain isn’t coming from misalignment — it’s coming from long-standing muscular contraction and stress patterns; causing the misaligment. The tension and torsion of muscles pull joints into uncomfortable positions which can create wear and tear on the delicate cartilage of joint surfaces. Shiatsu trains muscle tension patterns out of the body (with regular treatments) but you will feel noticeable relief from the first session. Then we work to stabilise your body (and mind) into a less tense state!
And this is where shiatsu is remarkably effective.
When to Choose Cranial Osteopathy
Cranial osteopathy may be the best option if you are:
Highly sensitive or easily overstimulated
Recovering from shock or trauma
Experiencing migraines or head tension
Seeking gentle nervous system regulation
Wanting very subtle intervention
It is a refined and powerful therapy. I continue to hold it in high regard and will sign post clients if this is required instead. Shiatsu is gentle but powerful and can also support you to release shock and trauma; as I have many clients with this. Migraines I have great success in alleviating from the first treatment.
When to Choose Shiatsu
Shiatsu is perfect for you if you are:
Living with chronic muscular tension
Experiencing joint pain linked to tightness
Feeling physically restricted or “held”
Carrying stress in your shoulders, hips or lower back
Noticing stiffness that isn’t resolving
Feel 'not right' even though the Dr says nothing is wrong
Take painkillers but they have no effect
As shiatsu is working on your body's energy patterns it can have fantastic results for pain that won't shift and there is a very simple explanation for this. Painkillers reduce heat and inflammation; this is called an acute pain and is usually temporary. There after is pain persists it becomes chronic and the issue is 'stagnation'. You may have held the joint/limb and it's become 'frozen'. The lack of movement reduces circulation and muscles fibres/fascia can dry out or lock or just hold onto the trauma and emotion of the situation. Western medicine doesn't recognise this but shiatsu does; and can effectively relieve.
Cranial osteopaths in Gloucestershire are often extremely busy. If you’re waiting weeks or months while continuing to live with discomfort, it’s worth considering shiatsu as an alternative or complementary approach.
Sometimes the body simply needs a more direct conversation through the muscles and connective tissue. It is well documentated now that trauma is held in the fascia (see blog on this) and shiatsu taps into this extensively.
Cranial Osteopathy or Shiatsu? There Is Space for Both
This isn’t about declaring one therapy better than the other.
Cranial osteopathy works from the inside out — supporting intrinsic rhythms and nervous system regulation.
Shiatsu works from the outside in — releasing muscular tension and improving circulation so the body can repair more efficiently.
Both aim to restore balance.
If pain is being driven by soft tissue restriction, shiatsu can succeed where the subtler approache of cranial rhythms may not .
My knees taught me this!
Both treatments are performed on a couch and fully clothed.
